INSTRUCTIONS: CALIFORNIA SCHEDULE A, RECEIPTS — SIMPLIFIED ACCOUNT (Form GC-405(A))
The form discussed in this article is used to document receipts of noncapital items by the estate of a conservatee or ward. This document can be obtained from the website maintained by the California Judicial System.
California Schedule A, Receipts — Simplified Account GC-405(A) Step 1: If filing for a conservatorship, check the first box.
California Schedule A, Receipts — Simplified Account GC-405(A) Step 2: If filing for a guardianship, check the second box.
California Schedule A, Receipts — Simplified Account GC-405(A) Step 3: Enter the name of the conservatee or minor.

California Schedule A, Receipts — Simplified Account GC-405(A) Step 9: In the third column of the table, enter the amount of each receipt.
California Schedule A, Receipts — Simplified Account GC-405(A) Step 10: You may complete up to four additional pages to document all receipts. However, if your documented receipts are in excess of five pages, you cannot use this document and must use a Standard Account format, which is found on form GC-400(A).
