INSTRUCTIONS: CALIFORNIA SCHEDULE C, DISBURSEMENTS — SIMPLIFIED ACCOUNT (Form GC-405(C))

The document discussed in this article is used to document payments made from the California estate of a conservatee or ward. This document may be obtained from the website maintained by the California Judicial Branch.

California Schedule C, Disbursements — Simplified Account GC-405(C) Step 1: Check the first box if filing for a conservatorship.

California Schedule C, Disbursements — Simplified Account GC-405(C) Step 2: Check the second box if filing for a guardianship.

California Schedule C, Disbursements — Simplified Account GC-405(C) Step 3: Enter the name of the conservatee or minor. Indicate which they are with a check mark.

California Schedule C, Disbursements — Simplified Account GC-405(C) Step 6: In the second column, enter the month, date and year of each order authorizing each payment. Leave blank every payment that was not authorized by a court order. A court order is not required to make a payment.

California Schedule C, Disbursements — Simplified Account GC-405(C) Step 10: You may attach up to four additional pages to document all disbursements. However, note that if your schedule exceeds five pages, you are required to file a standard account document using form GC-400(C).
