INSTRUCTIONS: CALIFORNIA SCHEDULE A, RECEIPTS, DIVIDENDS — STANDARD ACCOUNT (Form GC-400(A)(1))

The form discussed in this article is used by California guardianships and conservatorships to document receipts of dividends. Note that you should note returns of principal but not include their dollar amounts. Report returns of principal on Schedule A1, Return of Principal Items, instead, add their dollar amounts to cash assets on hand and subtract their dollar amounts from the carry values of the securities involved. Both documents can be obtained from the website maintained by the California Judicial Branch.
