The form discussed in this article is served upon a California person whose may be unable to take care of themselves and may require being placed into a conservatorship or limited conservatorship. This document can be obtained from the website of the California Judicial Branch.

California Citation For Conservatorship GC-320 Step 1: In the first box at the top of the page, the petitioner should enter the name of their attorney, as well as their state bar number, address, telephone number, and the name of the client represented. Entering their fax number and email address is optional.
