INSTRUCTIONS: NEW HAMPSHIRE FINANCIAL AFFIDAVIT (Form NHJB-2065-FS)

Both parties in a divorce, dissolution or legal separation must complete a financial affidavit. This document, known as Form NHJB-2065-FS, can be obtained from the website of the New Hampshire Judiciary.

New Hampshire Financial Affidavit NJB-2065-FS Step 1: At the top of the page, enter the case and court name at which your divorce, separation or dissolution is being processed.

New Hampshire Financial Affidavit NJB-2065-FS Step 2: In section one, enter all information requested about yourself.

New Hampshire Financial Affidavit NJB-2065-FS Step 3: In section two, list all children that are part of your marriage and state who they currently reside with.

New Hampshire Financial Affidavit NJB-2065-FS Step 4: In section three, provide the name and address of your employer, along with the date and place of your last employment.

New Hampshire Financial Affidavit NJB-2065-FS Step 5: In section four, list income from government benefits and similar programs.

New Hampshire Financial Affidavit NJB-2065-FS Step 6: In section five, detail all other monthly sources of income, including wages, trust income and disability benefits.

New Hampshire Financial Affidavit NJB-2065-FS Step 7: In section six, detail the monthly expenses listed, including health care and day care for children, court ordered support payments and state tax and mandatory pension contributions.

New Hampshire Financial Affidavit NJB-2065-FS Step 8: In section seven, list all assets, including their fair market value and the debt owed on them. Detail any additional assets in section eight.
