INSTRUCTIONS: PETITION FOR CHANGE OF NAME OF AN ADULT NV Packet 5
You can use this packet if you’ve lived in Clark County for at least 6 months before filing the Petition, you are at least 18 years old, and you have never been convicted of a felony. If you are only changing your middle name, you do not need to go through the court process.

You need to prepare and complete the following documents:
· Family Court Cover Sheet
· Petition
· Notice
· Order
· Request for Summary Disposition (if applicable)
· Affidavit of Petitioner (if applicable)

After completing the Petition and Notice of Petition, you need to make three copies of each. Stamp or write “Original” on each original document and take all documents to your local Clerk’s Office. You will need to pay the filing fees, and these fees range from county to county. You can pay by cash, money order, cashier’s check, or a personal check (if you have Nevada Driver’s license).

After the Clerk has stamped your copies, you need to publish the Notice with a local newspaper (preferably the Nevada Legal News, Review-Journal, or Sun. The Notice needs published once per week for three consecutive weeks.

Next, either request a Summary Disposition or request a “Prove-Up” hearing. Each method requires the petitioner to wait at least 11 days after the last Notice was published in the newspaper. If you’re requesting a summary disposition, you need to complete the Request and Affidavit. Take the completed forms the Clerk’s Office.
The staff for the judge will review the documents. If the staff approves the name change, the judge will sign the Order right there.

If you want a “Prove-Up” hearing, you’ll have to schedule a hearing with the Clerk. Bring all copies of your forms to the hearing and be prepared to answer questions in front of the judge. In either case, you will receive a signed Order in the mail. Mail a certified copy of the Order to the Department of Vital Statistics with a check for $20.00.
