INSTRUCTIONS: PETITION FOR CHANGE OF NAME OF A MINOR (NV Packet 7)

A petitioner can use these forms if they have a child or children in Clark County and the children have lived in the state for at least 6 months. This form is for the name change of children—not adults. You cannot use this form is a minor has been convicted of a felony.

The forms include in this packet include the following:

· Family Court Cover Sheet

· Petition for Change of Name

· Application for Appointment as Guardian Ad Litem (if applicable)

· Order Appointing Guardian Ad Litem (if applicable)

· Consent for Change of Name

· Notice of Petition for Change of Name

· Order for Change of Name

Other forms are required for requesting a trial and publishing notification about the name change.

Regard pages 6 through 13 of the packet for instructions on filling out each form. After completing the forms, make three copies of the Petition and Notice of Petition and file the forms with the Clerk’s Office.

You’ll either need consent from the non-petitioning parent for the change of name or you need to request appointment to guardian ad litem. In some cases, neither form is needed, but regard pages 14 and 15 of the packet for more information.

Notice of the name change needs published with a newspaper once a week for three consecutive weeks prior to the Summary Disposition or the Prove-Up trial. If such publication would endanger the minor child, ask the court to waive the requirement.

You can obtain an Order for the name change through a Summary Disposition or a Prove-Up hearing. For instructions on the Summary Disposition, regard pages 16 through 19 of the packet. For information on the Prove-up hearing, regard pages 19 through 21 of the packet.

Obtain copies of the Order from the Clerk’s Office for changing the minor’s important records.
