INSTRUCTIONS: MICHIGAN WAIVER/CONSENT (Form PC 561)

The form discussed in this article is filed by a person with interest in a Michigan conservatorship or guardianship appointment proceeding. This document can be obtained from the website maintained by the Michigan Courts system.

Michigan Waiver/Consent PC 561 Step 1: In the blank box at the top right hand corner, enter the file number.

Michigan Waiver/Consent PC 561 Step 2: On the first blank line, enter the name of the person whose conservatorship or guardianship is being considered.

Michigan Waiver/Consent PC 561 Step 3: On line 1, enter the reason for your interest in the guardianship or conservatorship proceedings.

Michigan Waiver/Consent PC 561 Step 4: Check the box on line 2 if you agree to waive notice of any hearings and consent to an application or petition of which you have received a copy. On the blank lines provided, specify the nature of the application or petition and the name of the applicant or petitioner.

Michigan Waiver/Consent PC 561 Step 5: Check the box on line 3 if you agree to waive notice of a hearing. On the blank line where indicated, specify the nature of the hearing which you are waiving notice of.

Michigan Waiver/Consent PC 561 Step 6: On the next blank line, enter the date.

Michigan Waiver/Consent PC 561 Step 7: On the next blank line, enter your signature.

Michigan Waiver/Consent PC 561 Step 8: On the first blank line of the left side of the bottom of the page, type or print the name of your attorney and their bar number.

Michigan Waiver/Consent PC 561 Step 9: On the second blank line, enter the street address of your attorney.

Michigan Waiver/Consent PC 561 Step 10: On the third blank line, enter the city, state, zip code and telephone number (including the area code) of your attorney.

Michigan Waiver/Consent PC 561 Step 11: On the first blank line of the right side of the bottom of the page, type or print or your name.

Michigan Waiver/Consent PC 561 Step 12: On the second blank line, enter your street address.

Michigan Waiver/Consent PC 561 Step 13: On the third blank line, enter your city, state, zip code and telephone number, including the area code.
