INSTRUCTIONS: MICHIGAN REGISTER'S STATEMENT (Form PC 568)

Michigan Register's Statement PC 568 Step 1: Enter the file number where indicated.

Michigan Register's Statement PC 568 Step 2: On the first blank line, enter the name of the person whose estate is concerned.

Michigan Register's Statement PC 568 Step 8: Check the box below if an authenticated copy of the will and codicils which has been probated is offered for informal proceedings and documents establishing probate in another state are in your possession. Enter the county and state in which these proceedings took place.

Michigan Register's Statement PC 568 Step 9: Check the box on line 2f if a will to which the requested appointment relates has been formally or informally probated.

Michigan Register's Statement PC 568 Step 10: Check the box on line 2g if either of the listed statements is applicable. Indicate which with a check mark.

Michigan Register's Statement PC 568 Step 11: Check the box on line 2h if there is good cause to reopen a previously administered estate and appoint a personal representative.

Michigan Register's Statement PC 568 Step 12: Check the box if a will is admitted to informal probate. Enter the date of the will and any codicils.
