INSTRUCTIONS: NEVADA COMPLAINT FOR ANNULMENT (Form D-11)

In Nevada, either spouse may file a complaint for annulment if they were married while intoxicated or otherwise while unable to give informed consent, if one party is insane, if one party engaged in fraud and provided a dishonest basis for the marriage, if bigamy is discovered, and under several other circumstances. This article discusses the forms which should be filed for an annulment in Washoe County, Nevada. These forms are available on the Washoe County courts website as one package.

Nevada Complaint For Annulment D-11 Step 1: The first document in this package is a cover sheet summarizing your case that will be submitted with your complaint.

Nevada Complaint For Annulment D-11 Step 2: The next document is a confidential family court information sheet that will not be part of the public record.

Nevada Complaint For Annulment D-11 Step 3: The next document is the actual complaint for annulment. Enter your name, address and telephone number in the top left corner where indicated.
