INSTRUCTIONS: 2011 COLORADO PROPERTY TAX/RENT/HEAT REBATE APPLICATION (Form DR 104 PTC)
Full-time Colorado residents whose total income is less than $12,313 if single and $16.025 if married are eligible for a property tax, rent or heat rebate as long as they are also 65 or older, 58 or older and a surviving spouse, or a disabled person of any age. You must also have paid your taxes for rent, heat and property to be eligible and cannot be a dependant on someone else's form. This document can be found on the website of Colorado.
2011 Colorado Property Tax/Rent/Heat Rebate Application DR 104 PTC Step 1: At the top of the form enter your name, date of birth and Social Security number, as well as that of your spouse if filing jointly. Note if either spouse is deceased with a check mark.

Line 12 asks if you paid rent for a property tax exempt unit and if so how much. Line 13 asks whether heat was included in your rent payments and if so the amount. Sign and date the form.
