Property 82179PT Step 1: Indicate with a check mark whether you are requesting a review based on valuation or classification from your county assessor, a review based on valuation or classification from the department of revenue, or a review based on an error in the tax rate assessed from the county board of supervisors.
Arizona Taxpayer Notice Of Claim - Personal Property 82179PT Step 2: Enter the date on which you are filing this document. If you are mailing this document, you must use certified mail.

Arizona Taxpayer Notice Of Claim - Personal Property 82179PT Step 7: In box 4, enter the basis for your claim and the correction you are requesting. On the right, enter the pertinent tax year, legal classification, assessment ratio and full cash value of the property in question.

attorney completing the form should give their name and address.
