INSTRUCTIONS: WISCONSIN INCOME TAX LONG-FORM (Form 1)
Wisconsin residents can use a long-form form 1 to file their state income taxes. The form can be found on the website of the Wisconsin Department of Revenue.

Income Tax Long-Form 1 Step 1: Enter the beginning and ending dates of your fiscal year if not filing on a calendar year basis.

Tax Long-Form 1 Step 2: Enter your
Social Security number. If filing jointly with your spouse, enter theirs as well.
Wisconsin Income Tax Long-Form 1 Step 3: Enter your home address, and give your tax district's county name and school district number.
Wisconsin Income Tax Long-Form 1 Step 4: Indicate your filing status with a check mark. If married but filing separate returns, enter your spouse's legal first and last name where indicated. Wisconsin residents with Minnesota income should document this where indicated.
Wisconsin Income Tax Long-Form 1 Step 5: Computer your Wisconsin income as directed on lines 1 through 13.
Wisconsin Income Tax Long-Form 1 Step 6: Lines 14 through 40 adjust your tax
liability on the basis of deductions, exemptions, credits and other subtractions. If claiming an itemized deduction, you will first need to complete Schedule 1 on the fourth page. Transfer the value from line 9 there to line line 20 on the second page. Similarly, complete Schedule 2 if claiming a credit for married couples in which both spouses are employed on line 30.
Wisconsin Income Tax Long-Form 1 Step 7: Lines 41 through 55 provide instructions for calculating your final tax owed or refund due. If you have previously underpaid, be sure to calculate your underpayment interest.
Wisconsin Income Tax Long-Form 1 Step 8: If you wish to authorize a third party to discuss this return with the Wisconsin Department of Revenue, check the box where indicated and give their name, phone number and personal identification number.
Wisconsin Income Tax Long-Form 1 Step 9: Sign and date the form and provide your daytime phone number. Your spouse should also sign if filing jointly.
Wisconsin Income Tax Long-Form 1 Step 10: Three different mailing addresses are provided at the bottom of the return. One is for those who owe tax, another is for those who are owed a refund or who owe no tax, and the last is for those claiming a homestead credit.
