INSTRUCTIONS: WASHINGTON D.C. NOTICE OF INITIAL HEARING PURSUANT TO SUPERIOR COURT, PROBATE DIVISION RULE 325
The form discussed in this article is used to give notice of an initial hearing to the subject for whom a petition to appoint a guardian or conservator has been filed in Washington D.C. This document can be obtained from the website maintained by the District of Columbia Courts system.
Washington D.C. Notice Of Initial Hearing Pursuant to Superior Court, Probate Division Rule 325 Step 1: On the first blank line, enter the name of the adult who is the subject of the petition.

If so, indicate with a check mark whether petitioning for the appointment of a general or limited guardian, and whether the individual is incapacitated because of mental retardation or because of other reasons.
