INSTRUCTIONS: WASHINGTON D.C. INVENTORY REPORT
The form discussed in this article is filed by a Washington D.C. guardian, conservator, committee, trustee or receiver for inventory purposes. This document can be obtained from the website maintained by the Washington D.C. court system.
Washington D.C. Inventory Report Step 1: On the first blank line under "Inventory Report," enter your name.
Washington D.C. Inventory Report Step 2: Indicate your position by crossing out all inapplicable positions listed.
Washington D.C. Inventory Report Step 3: On the next blank line, enter the date on which you were appointed to this position.
Washington D.C. Inventory Report Step 4: On the next blank line, enter the month in which you were appointed to this position.
Washington D.C. Inventory Report Step 5: On the next blank line, enter the year in which you were appointed to this position.
Washington D.C. Inventory Report Step 6: The first table provided is for documentation of real estate. Enter the location and description of each item in the first column, its assessed value in the second column, encumbrances and mortgages in the third column, and the market value in the fourth column. Provide the total of the second, third and fourth columns at the bottom of the table.
Washington D.C. Inventory Report Step 7: The next table provided is for documentation of bank deposits and other money. Enter the last four digits of each account number in the first column, the amount in each account in the second column, the name of the depository in the third column and the name registered on the account in the fourth column.
Washington D.C. Inventory Report Step 8: The next table provided is for documentation of notes secured by mortgage or deed of trust on real estate. Enter the location and a description of each piece of property in the first column, the balance due in the second column, the interest rate in the third column, the maker's name and the name of the present payor in the fourth column, the maturity in the fifth column and the terms of repayment in the last column.
