INSTRUCTIONS: WASHINGTON D.C. ACCEPTANCE

The form discussed in this article is used to accept appointment to a position as a Washington D.C. guardian or conservator. The form is filed as part of the process of obtaining Letters of Conservatorship and/or Guardianship from the court system. Both documents can be obtained from the website maintained by the District of Columbia Courts system.

Washington D.C. Acceptance Step 1: On the first blank line, enter the name of the adult who is the subject of this appointment.

Washington D.C. Acceptance Step 2: Check the first box if this form concerns appointment of a guardian.

Washington D.C. Acceptance Step 3: Check the second box if this form concerns appointment of a conservator.

Washington D.C. Acceptance Step 4: Check the third box if accepting appointment to the position of guardian.

Washington D.C. Acceptance Step 5: Check the fourth box if accepting appointment to the position of conservator.

Washington D.C. Acceptance Step 6: On the next blank line, enter the name of the ward.

Washington D.C. Acceptance Step 7: The subsequent paragraph states that by signing this document, you are consenting to personal jurisdiction in any action instituted in the District of Columbia by any interested person against you in your position as either guardian or conservator. The paragraph also states that if you are a non-resident, you irrevocably appoint the Register of Wills and successors to their office as the person upon whom all notice and process issued by a competent court in the District of Columbia may be served with the same effect as personal service in relation to all suits or matters pertaining to the proceeding in which Letters of guardianship and/or conservatorship shall issue.

Washington D.C. Acceptance Step 8: On the next blank line, provide your signature.

Washington D.C. Acceptance Step 9: On the next blank line, type your name.

Washington D.C. Acceptance Step 10: On the next blank line, enter your street address. Providing a P.O. box number is not acceptable.

Washington D.C. Acceptance Step 11: On the next blank line, enter your city, state and zip code.

Washington D.C. Acceptance Step 12: On the last blank line, enter your telephone number, including the area code.
