INSTRUCTIONS: APPLICATION FOR CHANGE OF NAME (Minor) CC-1427

This application is only used for the name change of a minor. The form must be filled out by a parent, legal guardian, or legal custodian. The state of Virginia advises all applicants to hire an attorney to help with this form, send notification to interested parties, and more.

Check the appropriate box on line 2 and write the county or city where the minor child currently resides. The Clerk of Court will provide the case number.

“Application for Change of Name (Minor) CC-1427 Step 2”

Provide the current full name of the minor child on line 3. Check the appropriate box in part 1 and provide any necessary documentation of a former name change.

“Application for Change of Name (Minor) CC-1427 Step 3”

In part 2, provide the full name of the applicant, the applicant’s address, and the applicant’s mailing address if different from the residential address. DO NOT provide information for the minor child in this section.

“Application for Change of Name (Minor) CC-1427 Step 4”

Check the appropriate box in part 3. Then, provide the date of birth, place of birth, and city or county of residence for the minor child in parts 4 and 5. Provide an address in part 6 if the address is different from the applicant’s address.

In parts 7 and 8, provide the full name and address of the father and mother of the minor child. Enter a mailing address if it’s different from the residential address of the mother or father.

Check the appropriate boxes in parts 9 through 11. If you check “Yes” in part 10, provide the name of the facility and the facility’s location. If you check “Yes” in part 11, provide the court name.

Provide the full current name of the minor child on the first line of the second page. Provide the full requested name of the minor child on the second line.

“Application for Change of Name (Minor) CC-1427 Step 8”

Sign this form in front of Notary Public. If the parents of the child are filing a joint application, the non-applicant needs to fill out the third part of the document.
