INSTRUCTIONS: UTAH ANNUAL STATEMENT FOR CONTINUED PROPERTY TAX EXEMPTION (Form PT-21)

Utah non-profit organizations which file a form PT-20 seeking property tax exemptions for real property used for religious, charitable or educational purposes can file a form PT-21 on an annual basis to extend this status. This form is available on the website of Utah's Property Tax division.

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 1: Under the section headed "general information," give the name of the organization. This name must be the recorded owner of the property in question.
