INSTRUCTIONS: UTAH APPLICATION FOR EXEMPTION - BENEFACTORS SCHEDULE C (Form PT-20C)
Utah nonprofit organizations which apply for property tax exemptions by filing a form PT-20 will also need to submit a Schedule C form separately. This document is available on the website maintained by the Property Tax Division of the Utah State Tax Commission.
Utah Application For Exemption - Benefactors Schedule C PT-20C Step 1: The first section asks for the name of the organization making the application, a contact person's name, a telephone number, the location of the property in question and the parcel or account number assigned.
Utah Application For Exemption - Benefactors Schedule C PT-20C Step 2: Question 1 under "Financial Information" asks whether use of the property in question generates any revenue, funds, products or services which are given away or sold. If yes, give the amount in dollars and provide a detailed description. If no, skip to step 6.
Utah Application For Exemption - Benefactors Schedule C PT-20C Step 3: If you answered "yes" to question 1, question 2a requires you to detail what percentage of funds, revenues, services and products are directly used for services for which nonprofit exemption status is claimed. Provide a written explanation of who receives the benefits and how they are chosen.
Utah Application For Exemption - Benefactors Schedule C PT-20C Step 4: Question 2b asks what percentage of funds, revenues, services and products are indirectly used for services that qualify for nonprofit exemption status. Describe the beneficiaries and how they are chosen.
Utah Application For Exemption - Benefactors Schedule C PT-20C Step 5: Question 2c asks what percentage of funds, revenues, services and products are given to shareholders or individuals. Provide a detailed, written explanation where indicated.
Utah Application For Exemption - Benefactors Schedule C PT-20C Step 6: Question 3 asks whether anyone receives wages, services or other benefits in return for performing services with the property in question. If so, you must attach an explanation of their compensation, how this compensation was set, their duties and working hours, and their relationship to the organization.
Utah Application For Exemption - Benefactors Schedule C PT-20C Step 7: Attach copies of all financial documents which provide proof of the nonprofit use of this property. Include documentation of benefits awarded to entities that did not take the form of revenue, products, services or funds. Sign and date the form.
