INSTRUCTIONS: AMENDED IOWA INDIVIDUAL INCOME TAX RETURN (Form IA 1040X)

Iowa residents use a form IA 1040X to amend their initially filed state 1040 form reporting income tax owed. Both of these documents can be found on the website of the government of Iowa.

Amended Iowa Individual Income Tax Return IA 1040X Step 1: Under "Step 1," provide your name, address and Social Security number. Provide your spouse's name and Social Security number if you file your taxes jointly.

Amended Iowa Individual Income Tax Return IA 1040X Step 2: Give the county and school district numbers of year residence at the end of the year being amended. Check the box where indicated if either you or your spouse was 65 or older in that year. Enter the calendar year for which you are filing.

Amended Iowa Individual Income Tax Return IA 1040X Step 3: Under "Step 2," mark the filing status of the six listed which applies to you. Check the box next to the statement explaining why you are filing an amended return.

Amended Iowa Individual Income Tax Return IA 1040X Step 4: Under "Step 3," detail your correct exemptions.

Amended Iowa Individual Income Tax Return IA 1040X Step 5: Under "Step 4," document your taxable income as directed. If filing jointly, enter your total income under column A. If filing separately, enter your information in column A and your spouse's in column B.

Amended Iowa Individual Income Tax Return IA 1040X Step 6: Under "Step 5," calculate your tax and credits as directed.

Amended Iowa Individual Income Tax Return IA 1040X Step 7: Under "Step 6," calculate any refund you are owed or any payment you must make.

Amended Iowa Individual Income Tax Return IA 1040X Step 8: On the second page, provide a detailed explanation for every reported change. Give the line number for every change and attach all applicable schedules documenting these changes. If married, provide an explanation of how income, deductions and credits have been divided between you and your spouse.

Amended Iowa Individual Income Tax Return IA 1040X Step 9: If you wish to change your credit carryford status, complete the bottom of the second page.

Amended Iowa Individual Income Tax Return IA 1040X Step 10: Sign and date the first page. If filing with your spouse, they must do the same. Provide a daytime telephone number.
