INSTRUCTIONS: WISCONSIN INCOME TAX (Form 1A)
Wisconsin residents document their income tax owed to the state with a form 1A. This document is found on the website maintained by the Wisconsin Department of Revenue, which also offers an instruction booklet you will need to complete this form.
Wisconsin Income Tax 1A Step 1: Enter your Social Security number at the top of the form. If you file taxes jointly with your spouse, provide their number as well.
Wisconsin Income Tax 1A Step 2: Give your name, as well as that of your spouse if applicable. Give your complete home address.
Wisconsin Income Tax 1A Step 3: Write the name of the city, village or town in which you reside. Give its county name and school district number.
Wisconsin Income Tax 1A Step 4: Indicate with a check mark if you are filing as a single person, as a married couple filing jointly, or as the head of household. If the latter, give the name of the dependant which qualifies you for this status.
Wisconsin Income Tax 1A Step 5: An unnumbered line is only for Wisconsin residents working in Minnesota.
Wisconsin Income Tax 1A Step 6: Lines 1 through 6 require you to document all sources of income and total them on line 7.
Wisconsin Income Tax 1A Step 7: Lines 8 through 10 concern deductions for your IRA, student loan interest or medical care. Total these on line 11. Subtract this sum from line 7 to determine your Wisconsin income and enter this on line 12.
Wisconsin Income Tax 1A Step 8: If you can be claimed as a dependent, check line 13 and complete the worksheet on page 11 of the instructions to fill in line 14. Otherwise, enter your standard deduction on line 14, which can be found on page 31 of the instructions. Subtract line 14 from line 12 and enter the result on line 15.
Wisconsin Income Tax 1A Step 9: Lines 16 through 24 provide instructions to calculate your net tax due. If both spouses are employed, complete the chart at the bottom of the second page to determine your married couple credit and enter the result on line 22.
Wisconsin Income Tax 1A Step 10: Lines 25 through 39 on the second page contain final instructions for determine the size of payment or refund due. Sign and date the form.
