INSTRUCTIONS: WISCONSIN INCOME TAX (Form 1)

Wisconsin residents use a form 1 to file their state income tax if they cannot use the short-form 1A form. Reasons for this include income which cannot be documented on a form 1A, such as that derived from a farm or business or adjustments such as alimony payments and tuition expenses. Both documents, as well as an instruction booklet for both, are found on the website of the Wisconsin Department of Revenue.

Wisconsin Income Tax 1 Step 1: If you are filing on a fiscal year basis, give your starting and ending dates.

Wisconsin Income Tax 1 Step 2: Enter your Social Security number, as well as that of your spouse if filing jointly. Provide your name and that of your spouse if filing jointly. Provide your complete home address.

Wisconsin Income Tax 1 Step 3: Enter the name of the city, village or district where you live, as well as its county name and school district code number.

Wisconsin Income Tax 1 Step 4: Indicate your filing status with a check mark where indicated. If you are married and filing separately, enter your spouse's name here rather than above.

Wisconsin Income Tax 1 Step 5: Wisconsin residents who earned income in Minnesota should note this on the line indicated and enter the amount of the income in question.

Wisconsin Income Tax 1 Step 8: Lines 14 through 40 on the second page provide further instructions for adjusting your income. Note that if you are claiming an itemized deduction on line 20, you must first complete schedule 1 on page 4. Married couples in which both spouses work must complete schedule 2 to claim the appropriate credit on line 30.

Wisconsin Income Tax 1 Step 9: Lines 41 through 55 on the third page provide the final instructions to calculate the amount of tax paid or the amount of the refund you are entitled to if you have overpaid.

Wisconsin Income Tax 1 Step 10: Sign and date the form. When filing, attach copies of your federal income tax returns and schedules.
