Apart from the Petition, you’ll also need to complete and file the following form:

· Order for Name Change of a Minor Child UJS034

· Notice of Entry of Order for Name Change of Minor Child UJS 035

The petition or other forms cannot be used if the other legal parent does not consent to the change of name. If you know the other parent will contest the change of name, you should speak with an attorney.

Complete the caption of the Petition. You need to file this form in the county the child currently resides.

In paragraphs 1, 2, and 3, enter the petitioner’s full name, the minor’s current legal name, and the child’s date of birth and gender.

In paragraphs 4 and 5, enter the location where the child was born and the petitioner’s relationship to the minor.

In paragraphs 6, provide the full name of the other parent and that parent’s last known address. In paragraph 7, provide the petitioner’s current address along with city and country.

In paragraph 8, name the county where you currently reside and how long you’ve lived there. In paragraph 9, provide how long the minor has been living at their current address.

In paragraph 10 provide where the child has lived for the last 5 years. Provide information about the mother and father in paragraphs 11 and 12. Enter the mother and father’s birthdates in paragraphs 13 and 14.

The rest of the form is simple to complete. Provide all necessary forms with the Clerk and they will schedule a hearing.
