INSTRUCTIONS: RHODE ISLAND SALES & USE TAX RETURN MONTHLY

Most Rhode Island businesses are required to file a sales and use tax return on a monthly basis even if no sales have been made. In certain cases, you may apply to the state's tax administrator for written authorization to file this return on a quarterly basis. Both types of forms are found on the website of the state's Division of Taxation. This form is not for use by non-retailer consumers, who should file a form T-205 instead to pay any use tax due on items purchased from out-of-state retailers on which state tax was not paid.

Rhode Island Sales & Use Tax Return Monthly Step 1: The first page of this document contains the form in triplicate. Separate all three copies by cutting along the dotted lines. Maintain one copy for your financial records and submit the other two.

Rhode Island Sales & Use Tax Return Monthly Step 2: On the first line of the form, enter the name of the business.

Rhode Island Sales & Use Tax Return Monthly Step 3: On the next two lines, enter the street address, city, state and zip code of the business.

Rhode Island Sales & Use Tax Return Monthly Step 4: On the next line, the owner, a partner in the business or an authorized agent should provide their signature.

Rhode Island Sales & Use Tax Return Monthly Step 5: Below this, the person signing should provide their job title and the date.

Rhode Island Sales & Use Tax Return Monthly Step 6: Below this, the person signing should provide their Federal Identification number and the end date of the month for which they are filing the return.

Rhode Island Sales & Use Tax Return Monthly Step 7: To the right, enter the net total of the sales and use tax owed and paid for this period. If no sales were made, enter "-0-" in this space.

Rhode Island Sales & Use Tax Return Monthly Step 8: This return must be filed by the 20th of the month following the month in question.

Rhode Island Sales & Use Tax Return Monthly Step 9: Some retailers may be required to submit their payments through an Electronic Funds Transfer (EFT). If you are unsure whether this requirement applies to your business, contact the phone number listed on the second page of this form.
