INSTRUCTIONS: RHODE ISLAND BUSINESS APPLICATION AND REGISTRATION (Form BAR)

Rhode Island businesses are required to apply for a sales permit. Those which are already in application must annually file the same form BAR to renew their permit by the fiscal year which ends on July 30. The form is found on the website maintained by Rhode Island's Department of Taxation.

Rhode Island Business Application And Registration BAR Step 1: The first four questions ask if you currently have employees working in the state, if you administer withholding on their wages for state taxes, if you lease employees in the state and if you make sales through retail. Note if your state tax liability is greater than $200 per month.

Rhode Island Business Application And Registration BAR Step 2: The next nine questions concern the type of products you will be selling. Note all additional fees which apply.

Rhode Island Business Application And Registration BAR Step 3: The last six questions of the first section ask about the operating dates of your business, as well as your location and business code numbers. These can be found on Instruction Sheet 1, which is included with this document.

Rhode Island Business Application And Registration BAR Step 4: Section A requires you to enter the type of business entity you are operating, your name, the business name and address, and all other identifying information requested. Provide a name and contact information for a person in charge of sales and payroll records.

Rhode Island Business Application And Registration BAR Step 5: In section B, give the name, address, Social Security number and title for every owner, partner or corporate officer. Attach additional sheets as necessary.

Rhode Island Business Application And Registration BAR Step 7: Section D-1 requires you to give a detailed description about your business activities, products, services and goods. Give the percentage each activity makes up of your business.
