INSTRUCTIONS: OKLAHOMA INDIVIDUAL HOUSEHOLD PERSONAL PROPERTY (Form OTC-935)
All Oklahoma residents are required to file a statement detailing their personal property with their county assessor on an annual basis. This should be filed between January 1 and March 15. If filed between March 15 and April 15, a ten percent penalty will be incurred. If you file this form OTC-935 after April 15, a twenty percent penalty will be incurred. This form can be obtained from the website of the Oklahoma Tax Commission. You may complete the form by printing or typing your responses.
Oklahoma Individual Household Personal Property OTC-935 Step 1: At the top of the page, enter the name of the county in which you are filing and the tax year for which you are filing.
Oklahoma Individual Household Personal Property OTC-935 Step 2: Enter your real estate and property account numbers where indicated.
Oklahoma Individual Household Personal Property OTC-935 Step 3: Give your name and complete mailing address, including the city, state and zip code.

Oklahoma Individual Household Personal Property OTC-935 Step 5: In part I, indicate with a check mark whether you own the real estate on which the personal property is located or whether you are renting or leasing.
Oklahoma Individual Household Personal Property OTC-935 Step 6: In Part II-A, give the value of any appraised improvement made to the property. Multiply this by ten percent . Below this, give the value of other miscellaneous personal property.
Oklahoma Individual Household Personal Property OTC-935 Step 7: In Part II-B, you should list all personal property such as furniture, appliances, lawn and garden implements, musical instruments, personal or professional libraries, watches, jewelry and recreational equipment. Give the item number, a description including the size and brand of the object in question, the date of acquisition, and the current fair market value.

Oklahoma Individual Household Personal Property OTC-935 Step 9: Sign and date the bottom of page two. If a paid preparer completed this form, they should sign the form and provide their identification number and contact information.
