Ohio Publication Of Notice 5.5 Step 1: Enter the name of the county in which the probate court is located on the first blank line.
Ohio Publication Of Notice 5.5 Step 2: Enter the name of the decedent whose estate is being probated on the second blank line.
Ohio Publication Of Notice 5.5 Step 3: Enter the case number assigned to administration of the estate on the third blank line.
Ohio Publication Of Notice 5.5 Step 4: Enter the name of the newspaper of general circulation in the county on the fourth blank line.
Ohio Publication Of Notice 5.5 Step 5: Check the first box if the notice should be published once.
Ohio Publication Of Notice 5.5 Step 6: Check the second box if the notice should be published once per week for two consecutive weeks.
Ohio Publication Of Notice 5.5 Step 7: Check the third box if the notice should be published once per week for three consecutive weeks.

Ohio Publication Of Notice 5.5 Step 14: Once publication has occurred, enter the dates of the editions in which notice was published on the first blank line on the second page, the cost on the second blank line, and the editor's signature on the third blank line.
