INSTRUCTIONS: OHIO EMPLOYEE'S WITHHOLDING EXEMPTION CERTIFICATE (Form IT 4)

Natural dependency exemptions such as the taxpayer in question and their spouse or children can use a form IT 4 to claim exemptions on taxes withheld from their wages in Ohio. This document can be obtained from the website of the Ohio Department of Taxation.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 1: On the first blank line, print your full name.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 2: On the second blank line, enter your Social Security number.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 3: On the third blank line, enter your home address and ZIP code.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 4: On the fourth blank line, enter your public school district of residence. If you do not have this information on hand, a URL is provided where you can look this up.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 5: On the fifth blank line, enter your school district number.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 6: On line 1, if you are claiming an exemption for yourself, enter the numeral "1."

Ohio Employee's Withholding Exemption Certificate IT 4 Step 7: On line 2, if you are married and claiming an exemption for your spouse, enter the numeral "2."

Ohio Employee's Withholding Exemption Certificate IT 4 Step 8: On line 3, if you are claiming exemptions for dependents, enter the number being claimed.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 9: On line 4, enter the sum of the numbers on lines 1 through 3.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 10: On line 5, enter the additional amount withheld per pay period as agreed with your employer if applicable.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 12: Enter the date on the last blank line. Detach the form from the top portion by cutting along the dotted line.

Ohio Employee's Withholding Exemption Certificate IT 4 Step 13: You may file a new certificate at any time if the number of the exemptions being claimed increases. Under circumstances defined on the top half of the page, if the number of exemptions decreases you must file a new certificate within 10 days.
