INSTRUCTIONS: OHIO INDIVIDUAL INCOME TAX RETURN (Form IT 1040)
Ohio residents can file their state income tax due using a form IT 1040. This document can be obtained from the website of the Ohio Department of Taxation.
Ohio Individual Income Tax Return IT 1040 Step 1: Enter your Social Security number and name, as well as that of your spouse if filing jointly.
