INSTRUCTIONS: OHIO APPOINTMENT OF APPRAISER (Form 3.0)
The form discussed in this article is used to appoint an administrator in an Ohio estate probate for the purpose of appraising assets of the decedent's estate which do not have a readily approvable document. As part of this appointment, the fiduciary agrees to pay the appraiser reasonable compensation for their services, subject to court approval on the amount of compensation. This document can be obtained from the website maintained by the Supreme Court of Ohio.
Ohio Appointment Of Appraiser 3.0 Step 1: Enter the name of the county in which the probate court is located on the first blank line.
Ohio Appointment Of Appraiser 3.0 Step 2: Enter the name of the decedent whose estate is being administered on the second blank line.
Ohio Appointment Of Appraiser 3.0 Step 3: Enter the case number assigned to administration of the estate on the third blank line.
Ohio Appointment Of Appraiser 3.0 Step 4: Enter the name of the appraiser on the fourth blank line.
Ohio Appointment Of Appraiser 3.0 Step 5: Enter the signature of the fiduciary or the applicant on the fifth blank line.
Ohio Appointment Of Appraiser 3.0 Step 6: The form should then be submitted to the probate court for approval. The next section concerns the hearing to be scheduled for approval of the appraiser. On the first blank line, the date of the hearing will be entered.

Ohio Appointment Of Appraiser 3.0 Step 8: On the third blank line, "A" will be entered if the hearing is scheduled for a.m. hours. "P" will be entered if the hearing is scheduled for p.m. hours.

Ohio Appointment Of Appraiser 3.0 Step 10: The probate judge will enter their signature on the fifth blank line.
Ohio Appointment Of Appraiser 3.0 Step 11: The last section will be completed when the appraiser is approved. On the first blank line, the date will be entered.
