INSTRUCTIONS: MONTANA DURABLE POWER OF ATTORNEY FOR HEALTH CARE AND MEDICAL TREATMENT
A power of attorney form regarding health care gives another person the right to make medical decisions on your behalf in the event that you are not capable of doing so yourself because of any physical or mental incapacity. A Montana Durable Power of Attorney for Health Care and Medical Treatment form must be completed to give someone else this authority.
