As part of a packet made available by Montana State University Extension on their website concerning accessing a deceased person's financial accounts, an affidavit that can be used to collect the decedent's personal property is included on the fourth page.

Montana Affidavit For Collection Of Personal Property Of The Decedent Step 1: The entirety of the form should be completed in the presence of a notary public. Enter the name of the county on the first blank line.
