INSTRUCTIONS: APPLICATION FOR EXTENSION OF TIME TO FILE MO-60
This form is due before the date of the tax return. If you’ve been approved for a federal extension and you don’t owe anymore income tax or you believe you’ll receive a refund, you do you not have to file this form. If an individual filer completes this form, automatic extension is granted until October 15, 2012. Fiduciary and partnerships are granted an extension until September 17, 2012.

It’s important to note that being granted an extension on filing an income tax return does not include an extension for the payment of the tax unless you receive an extension on time to pay. An interest of 5% or more is added to late payments.
“Application for Extension of Time to File MO-60 Step 2”
The filer needs to provide their name, spouse’s name (if applicable), they name of their attorney, address, and telephone number at the top of this form. Then, check the appropriate box to specify the type of extension. Only check one box. A separate MO-60 form is required for each type of tax return or extension.

Enter the taxpayer’s social security number of federal employer identification number depending on the type of return or extension.

Provide the tax year beginning, ending and extension in the next section. The, include the estimated tax liability for the taxable year on line 1.

On line 2, enter the sum of all payments and credits including any of the following:
· estimated tax returns
· withheld income tax
· credits from overpayment
· all tax credits
· income tax credits paid from another state
“Application for Extension of Time to File MO-60 Step 6”
For line 3, subtract line 2 from line 1 and enter the difference. The difference is the total amount of tax due.

Make a check or money order payable to Missouri Depart of Revenue. Complete the bottom of the form and send the completed form and fee to the following address:
