INSTRUCTIONS: APPLICATION FOR NAME CHANGE OF A MINOR (MN NAM202)
All forms need filed with the District Court in the County where the minor child currently resides. Apart from the Application, a Proposed Order Granted Change of a Minor (NAM107) and Criminal History Check Release (NAM103) are required. The criminal check is only required if the child is 10 or older.

Complete the heading of the form including the county, the current name of the minor, and the new name of the minor. Leave the court information blank.

List the current addresses of the minors, and check the box indicating you are a parent, legal guardian, or next of kin. The list the current legal names of the children and their dates of birth.

In part 5 and 6, provide as much information as possible. If the name and and/or address of the other parent is unknown, leave the space blank and check the appropriate box.

In part 9, provide a legal description of property the minor child has a claim, interest or lien. If you need help filling out this form, ask for the assistance of an attorney.

The applicant needs to provide a signature in both designated places on the form. If the minor is 14 years of age or older, they need to sign the form as well. Sign the form in front of Notary Public.

If the address of the non-applicant parent is known, you need to ask an attorney of steps you should take because every county is different. Some counties require mailed notice indicating the date, time, and place of the hearing, and other counties require personal service.

During the hearing, bring two adult witnesses who’ve known the minor for at least one year. Only bring one witness if both parents attend the hearing. Bring the child’s birth certificate. If the name change is approved, the judge will sign an Order Granting Name Change of Minor.
