INSTRUCTIONS: MASSACHUSETTS APPLICATION FOR STATUTORY EXEMPTION (Form 96)
Form 96 is a form filed by Massachusetts residents seeking a partial or total property tax exemption. Those who are widows or widowers, aged 70 or older (65 in some areas depending on the local assessor), the minor children of a deceased parent, veterans with a disability related to their service or blind may seek this exemption. The form is found on the website of the Massachusetts government.
Massachusetts Application For Statutory Exemption 96 Step 1: At the top of the page, give the name of your city or town, the fiscal year in question and your name and address.
Massachusetts Application For Statutory Exemption 96 Step 2: In section A, give your name, telephone number, marital status, address, mailing address (if different), the location of the property and the number of units.

If yes, indicate if you were a sole owner, co-owner with a spouse, or a co-owner with others. Note if it was subject to a trust on that date and if you have been granted exemptions for any other city or state this year. If so, name the location and give the size of the exemption.

Those who are blind should indicate this with a check mark and indicate if they are registered with the Massachusetts Commission of the Blind. If not, you must attach a letter from a physician confirming your blindness.
