INSTRUCTIONS: STATE OF ALABAMA MARRIAGE APPLICATION, BRIDE

Some Alabama counties require the bride and groom to file a separate marriage license application rather than completing a joint application. If you reside in such a county, both the bride and groom will need to complete these forms and submit them in person at the appropriate local courthouse. The form discussed here is valid in every county in Alabama where these joint applications are required and can be obtained from the website of the Madison County Probate Judge's Office.

State of Alabama Marriage Application, Bride Step 1: Enter your first, middle and last name. If you have a maiden name different from your current name, enter it where indicated.

State of Alabama Marriage Application, Bride Step 2: Enter your street address, along with your city, state, county name and zip code. Indicate yes or no to the question "Inside city limits" by circling the appropriate response. Write down your residence's phone number.

State of Alabama Marriage Application, Bride Step 3: Enter your birth date, including the month, day and year. Indicate whether your proof of age is a Military ID, driver's license, P+V or birth certificate by circling the appropriate response.

State of Alabama Marriage Application, Bride Step 4: Circle "White, " "Black," "American Indian," "Hispanic" or "Asian" to describe your race under the personal information. Underneath this, enter your Social Security number.

Numbers one through 12 indicate the last year of school education you completed, while numbers 13-16 indicate what year of the four years it takes to attain a bachelor's degree you completed.

State of Alabama Marriage Application, Bride Step 7: If you are under 18, you must have the consent of your guardian for the marriage to proceed. If this is the case, enter their full name, as well as the city and state where they reside.

State of Alabama Marriage Application, Bride Step 8: If you have been married previously, note this next to the line reading "Number of this marriage." You should also circle "Divorced," "Dissolution," "Death" or "Annulment" to indicate how your last marriage ended.
