INSTRUCTIONS: REQUEST FOR TAXPAYER IDENTIFICATION AND CERTIFICATION (Form W-9)
The W-9 form is not submitted directly to the IRS but is used by third party employers to correctly file all information regarding withholding income from your salary for tax purposes. It also helps establish that you are not subject to backup withholding of income related to unpaid taxes from previous fiscal years.
These instructions are based on the W-9 form provided by the IRS. This can be obtained from their website or in person at a local IRS office.
Request for Taxpayer Identification and Certification W-9 Step 1: Enter your name as it is written on your tax return. If you are completing the form for a business, enter that alternate name below.
Request for Taxpayer Identification and Certification W-9 Step 2: In the section provided to identify your classification under federal tax law, check the applicable box next to individual, corporation, S corporation, partnership, trust/estate, limited liability company (LLC). If you are exempt from withholding taxes, check that box instead.

Request for Taxpayer Identification and Certification W-9 Step 4: Enter the name and address of the person or business that has asked you to complete the form if you wish.
Request for Taxpayer Identification and Certification W-9 Step 5: In the section marked "Part I," enter your social security number if you are an American citizen filling the form out as an individual. Resident aliens should enter their Tax Identification Number (TIN). If you have not been issued one, write "applied for." Those who are completing the form as a sole proprietor or for an LLC that is a corporation or partnership, enter the Employer Identification Number (EIN) instead.
Request for Taxpayer Identification and Certification W-9 Step 6: In the section marked "Part II," you must certify that all three of the statements printed are true. Strike through item two if you are subject to backup withholding because the IRS has notified you have underreported interest and dividends on your tax returns.

Request for Taxpayer Identification and Certification W-9 Step 8: Submit the form to the person who has requested it. Maintain a copy of it for your financial records.
