INSTRUCTIONS: WAGE AND TAX STATEMENT 2011 (Form W-2)

Employees should be provided with a form W-2 documenting their wages and taxes withheld by their employer. A copy of this form is then filed with their individual income tax returns. This document can be obtained from the website of the Internal Revenue Service.

Wage And Tax Statement 2011 W-2 Step 1: In box a, enter the employee's Social Security number.

Wage And Tax Statement 2011 W-2 Step 3: In box c, enter the employer's name, address and zip code.

Wage And Tax Statement 2011 W-2 Step 4: In box d, enter the control number.

Wage And Tax Statement 2011 W-2 Step 5: In boxes e and f, enter the employee's name, address and zip code.

Wage And Tax Statement 2011 W-2 Step 6: In box 1, enter total wages, tips and other compensations.

Wage And Tax Statement 2011 W-2 Step 7: In box 2, enter the federal income tax withheld.

Wage And Tax Statement 2011 W-2 Step 8: In box 3, enter Social Security wages. In box 4, enter Social Security tax withheld.

Wage And Tax Statement 2011 W-2 Step 9: In box 5, enter Medicare wages and tips. In box 6, enter Medicare tax withheld.

Wage And Tax Statement 2011 W-2 Step 10: In box 7, enter Social Security tips. In box 8, enter allocated tips.

Wage And Tax Statement 2011 W-2 Step 11: Enter dependent benefits in box 10 and nonqualified plans in box 11.
