INSTRUCTIONS: IOWA RENT REIMBURSEMENT CLAIM FOR ELDERLY OR DISABLED PERSONS (Form 54-130a)
Iowa residents who are either 65 and older or permanently disabled may file a claim to receive reimbursement for part of their rent paid if they were full time residents of the state and currently live there. This form must be filed with the Department of Revenue and can be found on the website of the state of Iowa.
Iowa Rent Reimbursement Claim For Elderly Or Disabled Persons 54-130a Step 1: At the top of the page, enter your name, rental and mailing address, birth date and Social Security number. If you file taxes jointly with your spouse, enter their information here as well.
Iowa Rent Reimbursement Claim For Elderly Or Disabled Persons 54-130a Step 2: Check "yes" or "no" in response to line 1, which asks whether you were disabled as of December 31 of the previous year. Do the same for line 2, which asks about your disability status as of that date.
Iowa Rent Reimbursement Claim For Elderly Or Disabled Persons 54-130a Step 3: Line 3 asks whether you were a full time resident of the state in the previous year. Line 4 asks if you presently live in Iowa. Line 5 asks if you live in a nursing home.
Iowa Rent Reimbursement Claim For Elderly Or Disabled Persons 54-130a Step 4: Complete the worksheet on page 2. This requires you to total your income from wages, rent and utilities assistance, title 19 benefits for housing, Social Security income, disability income, income from pensions and annuities, interest and dividend income, profit from businesses owned and capital gains, money received from others living with you and any other income. Total your income on line K of the worksheet. If it is $21,906 or higher, you do not qualify for this reimbursement.
Iowa Rent Reimbursement Claim For Elderly Or Disabled Persons 54-130a Step 5: On line 6, enter your total from line K. Enter your rental period on line 7. Enter your total rent paid on line 8.
Iowa Rent Reimbursement Claim For Elderly Or Disabled Persons 54-130a Step 6: Perform all calculations as instructed on lines 9 through 12. This will provide your total reimbursement amount.
Iowa Rent Reimbursement Claim For Elderly Or Disabled Persons 54-130a Step 7: Give the name and address of your landlord, administrator or manager. Sign and date the form.
