INSTRUCTIONS: IOWA INDIVIDUAL INCOME TAX FORM (Form 2011 IA 1040)
Iowa residents or non-residents filing within the state may be required to file a long form tax return if they meet certain criteria. The form 2011 IA 1040 details who must file this form in the instructions that come with the document. This document is available on the website of Iowa's government.
Iowa Individual Income Tax Form 2011 IA 1040 Step 1: In section one, give your name, address and Social Security number. Include your spouse's name and Social Security number if applicable.
Iowa Individual Income Tax Form 2011 IA 1040 Step 2: On the right, give your county and school district number. The latter can be found in the instructions. Indicate whether your spouse is 65 or older. Note how many dependent children you have, if any, and indicate how many do and do not have health insurance.

Total all lines to determine your gross income on line 15.
