INSTRUCTIONS: REQUEST FOR COPIES OF HAWAII TAX RETURN (Form L-72)

If you require a copy of a previously filed Hawaii tax return, you may file a form L-72. The document can be obtained from the website of the state of Hawaii.

Request For Copies Of Hawaii Tax Return L-72 Step 1: Enter the date at the top right-hand corner.

Request For Copies Of Hawaii Tax Return L-72 Step 2: In box 1, print the name of the taxpayer as shown on the return in question. If filing on behalf of a business, enter their trade (dba) name.

Request For Copies Of Hawaii Tax Return L-72 Step 3: In box 2, give the taxpayer's current name and address. Indicate with a check mark if the name or address if different from your previously filed return. Enter the business and home telephone numbers of the requestor.

Request For Copies Of Hawaii Tax Return L-72 Step 6: In box 6, give the tax years or year end and period of the returns you are seeking copies of.

Request For Copies Of Hawaii Tax Return L-72 Step 7: In box 7, indicate with a check mark whether you are requesting regular or certified copies.

Request For Copies Of Hawaii Tax Return L-72 Step 8: If you wish for this copy to be mailed to someone other than yourself, give their name and address in box 8.
