INSTRUCTIONS: GEORGIA ST-3 ADDENDUM SALES

In Georgia, a ST-3 addendum is filed to document sales tax paid in multiple jurisdictions by a business which could not document all applicable sales tax in the space provided on form ST-3 itself. On this document, state sales tax and tax due to the cities of Atlanta, Clayton and Muscogee are computed. Lines 11 through 17 provide additional space for documentation. An ST-3 addendum is used to document business in additional jurisdictions. This document can be obtained from the website maintained by the Georgia Department of Revenue. Additional instructions are available in the form ST-3 packet also available on their website.

Georgia ST-3 Addendum Sales Step 1: Enter your sales and use tax number on the first blank line.

Georgia ST-3 Addendum Sales Step 2: Enter the end date of the period you are filing for on the second blank line.

Georgia ST-3 Addendum Sales Step 3: If this is an amended return, indicate this with a check mark.

Georgia ST-3 Addendum Sales Step 4: On the third blank line, enter your business name.

Georgia ST-3 Addendum Sales Step 5: On the next two blank lines, enter your complete address.

Georgia ST-3 Addendum Sales Step 8: In the third column, enter your taxable sales in each jurisdiction.

Georgia ST-3 Addendum Sales Step 9: In the fourth column, enter the sales tax rate of each jurisdiction.

Georgia ST-3 Addendum Sales Step 10: Multiply the value in the third column by the value in the fourth column. Enter the resulting product in the fifth column. This figure is your sales tax due.

Georgia ST-3 Addendum Sales Step 11: Total the values on lines 1 through 18. Enter the resulting sum on line 19. This is your total sales tax due from this addendum.

Georgia ST-3 Addendum Sales Step 12: Transfer the value from here to line 18 of Part B of form ST-3. Lines 1 through 18 will then be added and the resulting sum transferred to line 4 of Part A.
