INSTRUCTIONS: VERMONT FINANCIAL AFFIDAVIT: PROPERTY AND ASSETS (Form 813 B)

During any Vermont divorce involving children or in many other divorce cases, both spouses may be required to complete and submit a financial affidavit detailing their fiscal circumstances. This Form 813 B is available on the website of the Vermont Judicial System.

Vermont Financial Affidavit: Property And Assets 813B Step 1: At the top of the first page, enter the unit of your court, the docket number of your case, give your name and address, and indicate whether you are the plaintiff or defendant. Give the full name and date of birth of both parties as well.

Vermont Financial Affidavit: Property And Assets 813B Step 2: The bottom of the first page should be signed and dated in front of a notary public. This should only be done after you have completed the form.
