INSTRUCTIONS: NORTH CAROLINA FINANCIAL AFFIDAVIT (SHORT FORM) (Form 5)
In disputes over child support payments, alimony, attorney's fees and other divorce related fiscal issues, both spouses may be required to complete a statement detailing their finances. This article discusses the short form financial affidavit for North Carolina. Form 5 can be found on the website of the North Carolina Court System.
North Carolina Financial Affidavit (Short Form) 5 Step 1: Enter the number of the court hearing your case, the county and state at the top of the form. Enter the name of the petitioner and respondent, as well as the case number you have been assigned.
North Carolina Financial Affidavit (Short Form) 5 Step 2: Enter all information requested about your primary employer on the first page.
North Carolina Financial Affidavit (Short Form) 5 Step 3: On the second page, total all other sources of income. Next, total all deductions withheld from your wages. Subtract your deductions from your income to figure out your total net income.
