INSTRUCTIONS: FLORIDA PETITION FOR SIMPLIFIED DISSOLUTION OF MARRIAGE (Form 12.901(a))

In Florida divorcing couples without children who have completed a separation agreement and do not need a family court's ruling to any disputes may file a petition for simplified dissolution of marriage. This Form 12.901(a) must be filed along with separate financial affidavits (12902.(b) and (c)) and notices of Social Security number (12.902(j)) and a separation agreement (12.902(f)(3)). All these forms may be obtained from the website of the Florida courts system.
