INSTRUCTIONS: COLORADO SWORN FINANCIAL STATEMENT, SUPPORTING SCHEDULES (Form 1111S)

In divorces and other marital separations, both spouses must submit financial statements detailing all debts and assets. This Sworn Financial Statement, Form 1111, may need to be supplemented by a Supporting Schedules Form (1111S) to identify certain assets in detail. These documents should be submitted together and may be found on the website of the Colorado Court system.

Colorado Sworn Financial Statement, Supporting Schedules Form 1111S Step 1: At the top of the page, list the case name and number where indicated.

Colorado Sworn Financial Statement, Supporting Schedules Form 1111S Step 2: Under section F, list any stocks, bonds, mutual funds, securities and investment accounts which you possess. Provide the name of each of these, along with the number of shares you own, the last four digits of the account number and their value on the day you are filling out the form.

Colorado Sworn Financial Statement, Supporting Schedules Form 1111S Step 4: Under section G, list all pension, profit sharing or retirement funds by name. In the columns on the right, list the type of fund (such as an IRA or 401K), the last four digits of their account numbers and their value.

Colorado Sworn Financial Statement, Supporting Schedules Form 1111S Step 6: Section H is for you to list any "Miscellaneous Assets" not already listed. Some examples include frequent flyer miles, any loans you made which have not been repaid, stock options and memberships in organizations such as country clubs. List the value of each one on the right.

Colorado Sworn Financial Statement, Supporting Schedules Form 1111S Step 8: Under Section I, list all separate property which cannot be divided as part of a divorce. List their value and total them where indicated.

Colorado Sworn Financial Statement, Supporting Schedules Form 1111S Step 9: Take all total values and write them on Form 1111 where indicated. Submit the two statements together.
