INSTRUCTIONS: COLORADO SWORN FINANCIAL STATEMENT, SUPPORTING SCHEDULES (Form 1111S)

In divorces and other marital separations, both spouses must submit financial statements detailing all debts and assets. This Sworn Financial Statement, Form 1111, may need to be supplemented by a Supporting Schedules Form (1111S) to identify certain assets in detail. These documents should be submitted together and may be found on the website of the Colorado Court system.

Colorado Sworn Financial Statement, Supporting Schedules Form 1111S Step 1: At the top of the page, list the case name and number where indicated.

Colorado Sworn Financial Statement, Supporting Schedules Form 1111S Step 2: Under section F, list any stocks, bonds, mutual funds, securities and investment accounts which you possess. Provide the name of each of these, along with the number of shares you own, the last four digits of the account number and their value on the day you are filling out the form.

List the value of each one on the right.
