The form discussed in this article is filed by a Colorado guardian or conservator to provide notice of the death of their ward or protected person. This document can be obtained from the website maintained by the Colorado Judicial Branch.

Colorado Notice Of Death JDF 853 Step 1: Check the first box if filing in district court.

Colorado Notice Of Death JDF 853 Step 2: Check the second box if filing in Denver Probate court.

Colorado Notice Of Death JDF 853 Step 3: On the first blank line, enter the name of the county in which the court is located.

Colorado Notice Of Death JDF 853 Step 4: Where indicated, enter the court address.

Colorado Notice Of Death JDF 853 Step 5: In the second box, enter the name of the ward or protected person.

Colorado Notice Of Death JDF 853 Step 6: In the third box, enter the name of your attorney, as well as their address, phone number, fax number, email address and attorney registration number. If not represented by a lawyer, enter your own name, address, phone number, email address and fax number.

Colorado Notice Of Death JDF 853 Step 8: On the first blank line of line 1, enter the name of the minor or protected person.

Colorado Notice Of Death JDF 853 Step 9: On the second blank line, enter the date of death.

Colorado Notice Of Death JDF 853 Step 10: Indicate whether the person who died was the subject of a guardianship or conservatorship with a check mark.

Colorado Notice Of Death JDF 853 Step 11: Check the first box on line 2 if the guardian's authority to act on behalf of the ward has terminated.

Colorado Notice Of Death JDF 853 Step 12: Check the second box if the conservator's authority to act on behalf of the protected person is limited and the conservator will conclude administration of the estate.

Colorado Notice Of Death JDF 853 Step 13: On the next blank line, the conservator, guardian or their attorney should provide their signature and the date.
