Colorado Acceptance Of Office JDF 805 Step 1: In the first box, indicate whether you are filing this form in a district court or Denver Probate court with a check mark. Additionally, provide the county in which the court is located and its address.

Indicate which applies with a check mark. If so, skip line 5 through 9.
