INSTRUCTIONS: MONTANA CHILD SUPPORT GUIDELINES FINANCIAL AFFIDAVIT

During family court cases in Montana which concern minor children, you may be required to submit an affidavit documenting your fiscal circumstances. This document will help the court rule on issues concerning child support payments. The form can be found on Montana's official state website, in the section concerning the state's Department of Health and Human Services.

Montana Child Support Guidelines Financial Affidavit Step 1: In section A, give all personal information requested, including your name, address, contact information, education history and tax filing status.

Montana Child Support Guidelines Financial Affidavit Step 2: Section B concerns your children. Give all information requested about minor children, including their names and ages. Note any support you are already paying. Give all information requested about any health insurance you have through your job.
