INSTRUCTIONS: ARIZONA PROPOSED PATERNITY RESOLUTION STATEMENT (Form 5)
During custody and child support proceedings in Arizona, you may be required to file a proposed paternity resolution statement. This form five allows you to express your wishes concerning child support and custody arrangements in cases where two unmarried parents have a child.
Arizona Proposed Paternity Resolution Statement 5 Step 1: At the top of the page, enter your name, address, telephone number and indicate whether you are representing yourself or are an attorney acting on behalf of one of the parties.
Arizona Proposed Paternity Resolution Statement 5 Step 2: Enter the name of both parties, the case number, and indicate whether this form is being submitted on behalf of the father or mother.
Arizona Proposed Paternity Resolution Statement 5 Step 3: In section one, indicate with a check mark if you or your children receive public assistance, or whether you have another case pending with the Division of Child Support Enforcement.

Indicate how parenting time should be divided and whether joint or sole legal custody should be awarded. If the latter, indicate to which parent.

Arizona Proposed Paternity Resolution Statement 5 Step 7: In section five, if you wish for your children's names to be changed, write what you would like their new name to be.
